CUSTODIANSHIP DECLARATION FOR A MINOR STUDYING IN CANADA

Part A — Parents / guardians

We, [parent/guardian names], of [address, country], the [parents/guardians] of [minor’s full name], born [DD/MM/YYYY], DO SOLEMNLY DECLARE THAT we have appointed [custodian full name], of [address, city], Ontario, Canada, to act as custodian for our child while the child studies in Canada, and that we authorize the custodian to act in our place for the care and support of our child.

Part B — Custodian in Canada

I, [custodian full name], of [address], Ontario, DO SOLEMNLY DECLARE THAT I undertake to act as custodian for [minor’s full name] and to make arrangements for the care and support of the minor, and for the minor’s needs, while in Canada.

And we/I make this solemn declaration conscientiously believing it to be true and knowing that it is of the same force and effect as if made under oath.
